The standard form is printed in black on white paper, with a blue cardboard and silver-coloured metal 'seal' at the top left-hand corner, and three red wax seals on a strip of black ribbon at the bottom right-hand corner. The details are filled in with black ink, and signed by John Broxholm, his father Christopher, and William Bryan next to the wax seals. The frame is made of light brown wood with a glass front. The glass has cracked diagonally.
