[i.e. catalogue code of John Mitchell (ca. 1685-1751), librarian to Sir Richard Ellys]. Erased manuscript inscription in cartouche on front pastedown, probably sixteenth-century. Manuscript inscription at head of first leaf: "Ex dono [?] liberalitate mihi amantissimi Domini Caroli [?] doctoria medici parisiensia anno do[min]i 1620". Manuscript codes on rear pastedown, including: "5222 ... 1616 ..."[?]. Binding: sixteenth or seventeenth-century full parchment (stained and spattered) over very thin boards; sewn onto three supports; holes for two pairs of ties (missing); spine cracked, partially missing; printed waste lines the spine; printed waste as part of pasteboards visible underneath pastedowns.
